Client Care Coordinator – Lincoln

You will oversee a team of Caregivers, empowering and encouraging them to be their best, through educational and hands-on training, positive communication, and support. Additionally, you will foster a positive relationship with each client served by your Caregiver team, and at times serve as their Caregiver. As the first point of contact for your group of clients and Caregivers, you play a vital role in ensuring the communication happens that delivers positive client experiences.

Job Description
Salary: $17- $20 an hour
Job Type: Full time
Shift and Schedule: Monday – Friday, on call
Work setting: Onsite, Hybrid (After 90 days of Employment)

Care Coordinator Responsibilities
- Client and Caregiver Scheduling:
Develop and maintain schedules for clients and caregivers based on the client’s care needs, caregiver availability, and other factors. Coordinate and assign caregivers to clients, ensuring appropriate match based on skills, experience, and client preferences.
- Schedule Optimization:
Continuously review and adjust schedules to optimize caregiver utilization, minimize travel time, and maximize efficiency. Identify and address scheduling conflicts, gaps, or issues in a timely manner.
- Communication and Coordination:
Maintain ongoing communication with clients, caregivers, and the care team to ensure clear and accurate information exchange regarding scheduling, changes, and updates. Act as a central point of contact for scheduling-related inquiries and concerns.
- Caregiver Onboarding and Training:
Collaborate with the Recruiter and Care Team to onboard and train new caregivers, ensuring they are equipped with the necessary skills and knowledge to provide quality care. Coordinate and schedule training sessions as needed.
- Caregiver Availability and Leave Management:
Track and manage caregiver availability, time off, and leave requests. Ensure appropriate coverage is maintained during caregiver absences or vacations.
- Documentation and Reporting:
Maintain accurate records of client schedules, caregiver assignments, and any changes or updates. Generate reports and provide updates to the care team regarding scheduling metrics, trends, and challenges.
- Client Assessments and Care Plans:
Collaborate with the care team to conduct client assessments and develop individualized care plans. Incorporate care plan requirements into scheduling to ensure client needs are met.
- Quality Assurance:
Monitor and evaluate the quality-of-care services through regular check-ins with clients, caregivers, and the care team. Address any concerns or issues related to scheduling or caregiver performance promptly and appropriately.
Client Care Coordinator Qualifications
Education, Skills, and Experience
Minimum Requirement
A high school diploma or equivalent is required and three years’ experience in a related field or combination of education, experience, and training.
A valid and active Certified Nursing Assistant (CNA) or Medication, HHA License is required. Additional certifications, such as Basic Life Support (BLS) or specialized training in areas like behavioral challenges, dementia and Alzheimer’s are highly advantageous.
Preferred Qualification
- A bachelor’s degree in a related field
- Minimum 3 years’ experience preferred.
- Familiarity with medical terminology.
- Excellent communication skills and ability to provide a high level of customer service in a fast- paced environment.
- High degree of professionalism, attention to detail and effectiveness.
- Trouble- shooting, prioritizing and organizational skills essential.
- Experience with private residence in home care scheduling.
- Prior experience in healthcare scheduling, coordination, or a similar role is preferred. Experience working in a home care or senior care setting is beneficial.
- Strong organizational and multitasking abilities to manage multiple schedules and prioritize tasks effectively.
- Excellent communication and interpersonal skills to interact with clients, caregivers, and the care team.
- Proficiency in scheduling software or electronic scheduling systems.
- Attention to detail and accuracy in maintaining schedules and documentation.
- Problem-solving skills to address scheduling conflicts or challenges.
- Knowledge of care services and care plans to ensure appropriate caregiver assignments.
- Ability to work collaboratively in a team environment.
- Flexibility: Willingness to work flexible hours to accommodate scheduling needs, which may include evenings, weekends, and holidays.
Lifting Requirements
- Ability to participate in physical activity. Ability to work for extended period of time while standing and being involved in physical activity.
- Heavy lifting and transfers. Ability to do extensive bending, lifting and standing on a regular basis.
